I hereby authorize the repairs and/or services shown to be performed on the vehicle identified above, which may include the installation of parts and materials and/or teardown. I further authorize you to perform diagnostics and testing to ascertain the cause and correction of my complaint, when necessary, for which a charge may be incurred. I understand you will provide me with an estimate of charge for my approval for any work not specified on this check-in form. I further understand that should I decline recommended repairs and services, diagnostic/teardown charges incurred may be due. I agree to these charges without dispute. I hereby authorize you and/or your employee's permission to operate and drive my vehicle to the extent you deem necessary to inspect and/or test the vehicle. I acknowledge and agree that Katy Collision Center shall not be responsible or liable for any loss of the vehicle or any articles left in the vehicle. There will be a garage fee of $75 each day after signing.
You are hereby notified in accordance to the Texas Motor Vehicle Laws Section 70.001-70.008 that your vehicle may be subject to repossession for return checks such as insufficient funds, account closed, stop payment, or stolen checks. You will also be obliged to pay the cost of the repossession and storage as a condition of reclaiming the vehicle.

Thereby authorize the teardown of my vehicle to assist in estimation and expedite repairs and understand that additional fees/services could apply (ex: photos, etc.. And an Admin fee of ($495), Preservation fee of ($295), Teardown fee of ($555), Blueprint fee of ($425), Gate fee of ($250), and minimum transfer fee of ($150) will be assessed if the vehicle is removed without repair.
